nursing homes, institutions, or hospitals into independent community-based
settings. These person-centered services include housing search,
arranging home modifications, covering non-reoccurring expenses (deposits,
furniture), and setting up personal care.
Key Components of Community Transition Services:
- Person-Centered
Planning: Transition coordinators work directly with the
individual to identify their goals, preferences, and necessary support
systems for a successful move.
- Housing
Assistance: Navigators assist in finding affordable, accessible,
and safe housing options, including apartments or homes.
- Transition
Funding: CTS can cover one-time costs, such as security deposits,
utility initiation fees, and purchasing basic household goods or
furniture.
- Support
Coordination: CIL staff coordinate with Medicaid, nursing
facility staff, and community resources to arrange in-home support
services, personal care, and transportation.
- Home
Modifications: Assistance with installing necessary accessibility
features (e.g., ramps, grab bars) in the new home.
Eligibility & Goal:
The primary goal is to provide individuals with disabilities (regardless of
age) a, choice in living independently rather than in an institution. Eligible
participants are typically those residing in a nursing facility who are
Medicaid-eligible and wish to return to community living.
